The techniques of pharmacological induction of childbirth
1. The prostaglandins
"The prostaglandins - or rather a synthetic substance that mimics the natural chemical mediator - are manually applied vaginally in the form of gel or small strips of tissue which, little by little, are able to make the uterine neck mature, softening and smoothing it, to allow it to dilate easily »explains Professor Bianchi.
It is also possible to administer prostaglandins orally or sublingually, «currently it is the most used method in the world, and as such recommended by the World Health Organization. 2. Oxytocin

The double balloon
«At Buzzi - Ferrazzi specifies - when the uterine neck is still closed, but it is now necessary to resort to induction, a special double balloon is applied which remains in place for 12-18 hours and mechanically leads to neck dilation. The epidural with induced delivery
The induction of childbirth does not preclude the use ofepidural. "In some cases, if there are already very painful contractions, at the Buzzi in our city we begin to do it even before the active phase, that is, in the latent phase in which uterine contractions are still irregular" continues Enrico Ferrazzi.
The risks of induction methods
With both methods there is a risk of uterine hypertone: that is, the uterus, artificially solicited to modify itself, tends to remain contracted and this can compromise the oxygenation of the fetus.
Finally, in 20-25% of cases the induction may not be successful and require recourse to the caesarean. “If a 28-year-old woman is in her first pregnancy and the birth is spontaneous, at full term, she has a risk of a caesarean section of 6-8%. If the same woman undergoes an induction of childbirth, after 41 weeks, the risk of a caesarean increases to 15-20% because the placenta is "older" and risks not supporting the oxygenation of the fetus during labor »concludes Ferrazzi.
